If you’d like to transfer your dental records to McDaniel Family Dentistry, please complete our Records Release Form. Once you have completed it, please bring it by our office or email it to mcdanielfamilydentistry@gmail.com and we will try to obtain a copy of your records. This allows us to obtain your previous dental history, ensuring continuity of care and the best possible treatment. We look forward to serving your dental needs!
